Oologah-Talala Public Schools provides a centralized enrollment office offering convenience to parents enrolling children new to the district. The Enrollment Center is located in the center of our school campus directly south of the football field. You will access the Enrollment Center's entrance going west on 390 RD and head north on Mustang Drive. All new students must be a resident of the Oologah-Talala Public School district, living with their parent or legal guardian.
ITEMS TO BRING WITH YOU TO ENROLL
Mailing address: If you use a Post Office box as your mailing address, we must have your 911 address also.
Proof Of Residency: Acceptable proof includes one of the following: current, original, full page electric bill in the name of the care provider reflecting the service address (NO cut-off notices). CURRENT lease/rental agreement, or a Real Estate purchase contract (NO CONTIGENCY CLAUSE) reflecting the property address, agent's name, address and phone number.
An original state certified birth certificate.
Updated Immunization records from health department or doctor's office must be presented for students to enroll. Immunizations must be current and up to date.
Proof of Guardianship (if applicable). If the enrollee is not living with both birth parents you must provide proof of guardianship (court document or divorce decree).
Parent/Guardian driver's license.
Copy of transcript for 6th - 12th graders. Your student will not begin classes until a transcript is received. Address of previous school, phone number and fax number.
Other documents (if applicable): Certified Degree of Indian Blood (CDIB) Card, Special Education (IEP) or Gifted/Talented records. Important addresses and telephone numbers (i.e. place of employment, physician, emergency numbers).
Your child's enrollment will not be complete until all documents have been submitted.
