The OSSAA has changed the Physical Form as of April 25, 2024. We cannot accept any old forms for the 2024-2025 school year. If you have already filled out the old form for your student, a new form will need to be completed. We have updated the forms on both the school's website and Rank One. You may need to provide your doctor with the new form at the time of your student's physical, as no old forms will be accepted. As a reminder, you may have a physical completed at the OHS Gym on May 9 from 6:00 - 7:00. The cost is $25, and a new form will be provided for you.
